National Kidney Foundation of Michigan was awarded the 2015 Michigan Association of Health Plans Business Affiliate Award

ANN ARBOR, MICH.—(Sept. 18, 2015)— On Tuesday, Sept. 15 the National Kidney Foundation of Michigan was awarded the 2015 Business Affiliate – Business Practice Improvement award for their “Diabetes Prevention Program” by the Michigan Association of Health Plans (MAHP).

“Each submission represents the remarkable drive our MAHP members have to improve the health of those in their communities,” said Rick Murdock, executive director of MAHP. “A majority of these innovative practices are implemented by other plans, so these programs end up benefiting many more Michiganders than originally intended. It’s truly a testimony to the remarkable influence these programs have, and it’s our honor to recognize their achievements.”

The National Kidney Foundation of Michigan partnered with Priority Health to deliver the My Choice…My Health: Diabetes Prevention Program™ for at-risk plan members. The effort has reached 276 participants at ten different locations across Michigan. It’s a model to show how community-based organizations and health plans can work together to reduce chronic disease costs and improve the quality of life for people at risk.

 

Participants in My Choice…My Health: Diabetes Prevention Program™ are a part of a small, supportive group lead by a trained lifestyle coach. They learn how to adopt healthier eating habits, ways to be more physically active and other behavior changes over the course of 16 weekly one-hour sessions. The group continues meeting monthly, completing a full year of My Choice. . . My Health. The participants work towards losing 5-7% of their starting body weight and being physical active for 150 minutes per week.
