My Choice . . . My Health is the Diabetes Prevention Program of the National Kidney Foundation of Michigan. It helps those at high risk of developing type 2 diabetes adopt and maintain healthy lifestyles by eating healthier, increasing physical activity and losing a modest amount of weight in order to reduce their chances of developing the disease. Based on research funded by the National Institutes of Health and the Centers for Disease Control and Prevention, the program reduces the risk of developing type 2 diabetes by 58%.
In a classroom setting, a trained lifestyle coach facilitates a small group of participants in learning about healthier eating, physical activity and other behavior changes over the course of 16 weekly one-hour sessions. Topics covered include healthier eating, getting started with physical activity, overcoming stress, staying motivated, and more. After the initial 16 core sessions, participants meet monthly for added support to help them maintain their progress.

Insurance (I): This program is free to Priority Health members. Just show your member ID card!
Self-Pay (P): There is an up-front fee plus an additional fee per class. We ask you to commit to at least the first 5 sessions. For questions about your payment options, please attend an info session or email email@example.com.
Scholarships (S): Scholarships are available to qualifying participants. You must fill out an application. Contact firstname.lastname@example.org for an application.
Free to Qualified Participants (F): Fully funded by grants or community organizations.

Can I participate?
To qualify for the program, individuals must be overweight and at high risk for developing diabetes or overweight and have been diagnosed by a physician as someone with pre-diabetes. Take the CDC Prediabetes Test to determine your risk.
Am I at risk?
Your doctor can tell if you are at risk. He/she can determine your risk through a blood test with one of the following results:
- Fasting plasma glucose between 100-125 mg/d
- A1c between 5.7% and 6.4%
- 2-hour Plasma Glucose between 140-199 mg/dl
In addition, a combination of risk factors such as family history, overweight, and gestational diabetes may also put you at risk. Take the Centers for Disease Control Risk Test.
